ABSTRACT

Objective To investigate the current situation of radiological health technical service institutions and their technical services in China, and to provide a basis for better utilizing radiological health technical service resources and strengthening institutions’ capability building. Methods From October to December 2021, we conducted quality monitoring, ability comparison, capability building surveys, and other investigations on radiation health technical services to collect information on the qualifications and technical services of radiation health service institutions. The data were pooled and analyzed using SPSS software. Results By the end of 2021, there were 608 radiological health technical service institutions across China, with 47.0% of them located in the eastern region, and the percentage of institutions in the health system (42.6%) was less than that in the non-health system (57.4%). Institutions of grade A offered 62.3% of technical services, while institutions of grade B provided 37.7% of technical services; technical services provided by the non-health system and the health system accounted for 84.2% and 15.8%, respectively; institutions in the health system at the province, prefecture, and county levels offered 37.1%, 50.6%, and 12.3% of technical services, respectively. Conclusion In recent years, the proportion of institutions in the non-health system has increased significantly, but the proportion of institutions in the health system with radiological health qualifications is small, and the development is uneven in the numbers of institutions, technical services, and professional and technical personnel in the eastern, central, and western regions and at the province, prefecture, and county levels in China. It is necessary to further encourage and support health system institutions in strengthening capability building and applying for radiological health qualifications, and to increase construction and investment in the western region and at the district/county levels, in order to improve the overall capability and level of radiological health technical services in the country.

ABSTRACT

ObjectiveTo analyze the production of Poria in 5 major producing areas and the problems through field investigation and literature review and thus to lay a foundation for healthy development of Poria industry. MethodEach production process of Poria in Anhui, Hubei, Hunan, Yunnan, and Sichuan was probed and related research was retrieved for the analysis. ResultSichuan, Guangxi, Guizhou, and other areas have been the emerging producers of Poria. However, a few Poria cocos lines were used and the variety breeding lagged behind. Different cultivation techniques were adopted in different production areas. For example, the "induction" method failed to be widely used. Moreover, the harvest time was mainly dependent on the market demand not matter it was suitable or not. Furthermore, steaming has replaced the traditional diaphoretic processing in the processing of this medicinal material in production areas. In addition, fumigation with sulphur was still used in the processing of Poria. ConclusionAn excellent variety is the key to the quality of Poria. Efforts should be made to strengthen the evaluation of the germplasm resources of P. cocos and variety breeding and standardize the processing in production areas, thereby ensuring the safety and effectiveness of Poria and the decoction pieces. In addition, the contradiction between artificial cultivation and woodland ecology should be fully coordinated to ensure the sustainable development of Poria industry in China.

ABSTRACT

Objective:To describe the status quo and subtypes of postoperative delirium (POD) in children with congenital heart disease in China. Methods:A total of 216 children with congenital heart disease who underwent surgery in Fuwai Hospital, Chinese Academy of Medical Sciences from December 2020 to June 2021 were prospectively observed. The status quo and subtypes of POD in children with congenital heart disease was evaluated according to the results of the Cornell assessment of pediatric delirium (CAPD).Results:POD occurred in 114 children (52.78%) during the study period. The onset time of POD was most on the first postoperative day (50.88%). POD lasted only 1 day in 54.39% of the children who developed it. The incidence of POD subtypes (hyperactive delirium, hypoactive delirium, mixed delirium) were 35.09%, 29.82% and 35.09%, respectively, and there was no significant difference in the incidence distribution among them ( χ2=0.63, P>0.05). The onset time of mixed delirium was significantly later than that of hyperactive delirium ( χ2=7.07, P<0.05), and the duration of mixed delirium was significantly longer than that of hypoactive delirium ( χ2=16.81, P<0.001). The results showed that if the child had blood transfusion during operation or the operation was difficult, the probability of reduced activity delirium after operation was higher ( P<0.016 7). Children wieth moderate to severe postoperative pain or short duration of muscle relaxant use during the evaluation period were more likely to have a hyperactive postoperative delirium.( P<0.016 7). The time of tracheal intubation, the time of hospitalization after operation and the the postoperative time in PICU were the longest in the children with mixed type of postoperative delirium ( P<0.016 7). Conclusions:POD has a high incidence in children with congenital heart disease, and it is more likely to occur in the early postoperative stage. Therefore, early postoperative identification and intervention of POD is of great significance for children with congenital heart disease. Children with different characteristics are prone to different types of POD, which should be paid attention to.

ABSTRACT

New medical service price items (hereinafter referred to as the new item) management is key to deepening medical service price reform.Based on the review of the policy development of the new item management and status quo of provincial-level practices, the authors sorted out the problems in the new item management as follows. Namely the new item connotation was not clearly defined, linkage with the national specifications for medical service price items was not yet well defined, the approval process was imperfect, the pricing fundamentals were weak, and the maturity term system was to be strengthened. According to such analyses, the authors proposed countermeasure recommendations for optimizing the new item management: to introduce and implement unified national specifications for medical service price item and the new item guidance as quickly as possible, to promote the linkage between the new item management and the dynamic adjustment mechanism of medical service prices, and to strengthen institutional construction of the new item management.

ABSTRACT

@#Diabetic retinopathy is the most common disease of microvascular diseases caused by diabetes. It is harmful to vision in the late stage. If not treated in time, it can cause blindness. The diagnosis, treatment and follow-up of diabetic retinopathy were mainly based on fundus fluorescein angiography. There are some limitations in the clinical application because of the side effects of its operation, such as invasive, allergic and adverse reactions. Recently, optical coherence tomography has become more and more popular among ophthalmologists for its noninvasive, rapid and high resolution fundus angiography. It cannot only display the choroidal vascular network information in layers, but also quantitatively analyze the changes of blood flow in the macular region. For the first time, the blood flow analysis of optic disc and macular area in vivo has reached the level of tissue anatomy. It has wide application prospect and is more and more favored by ophthalmologists. In this paper, the clinical application of octa in diabetic retinopathy was reviewed.

ABSTRACT

OBJECTIVE: To systematically evaluate the status quo of cost estimation in pharmacy intravenous admixture services (PIVAS), and to provide cost basis for the construction of PIVAS in China. METHODS: Retrieved from PubMed, Embase, Cochrane library, CBM, CNKI, CSJD and Wanfang database from database establishment to Jan. 2019, the studies about the status quo of cost estimation in PIVAS of China were included. The descriptive analysis was conducted for content and method of cost estimation, infection to hospital. RESULTS: A total of 17 literatures were included, involving 8 before and after control studies, 6 experience sharing studies and 3 reviews. Existing reports showed that the estimation contents and methods of PIVAS cost were roughly the same. The cost included manpower, medical and health materials, fixed asset purchase, depreciation, repair costs, medicine cost and indirect costs. At the same time, the infection to hospital were reported, such as in manpower adopting, formulating detailed management measures and systems, concurrent allocation of the same kind of drugs, shortening infusion preparation and replacement time, in order to save manpower cost. CONCLUSIONS: PIVAS cost calculation method is roughly the same in some hospitals, but there is no uniform standard. It is necessary to further improve the PIVAS cost measurement standard and provide a basis for the construction and development of PIVAS in China.
